How to implement it?
As a part of the Digital Health & Wellness Summit, Linkcare presented its innovation in Collaborative Community Care (3C). Jose Sanchez explained how Linkcare allows health care professionals, caregivers, neighbours and patients get coordinated and more engaged in taking care of their health. 3C is becoming the most effective and viable way for providing care support for an ageing population having long term conditions.
